Most facilities are run by people trying hard with thin resources. The point of knowing the red flags isn't cynicism. It's knowing which buildings have stopped trying.

If you see red flags where someone already lives

Document what you saw with dates. Raise it with the administrator in writing. And know the escalation path: every state has a Long-Term Care Ombudsman, a free, independent advocate for residents (find yours via the Eldercare Locator, 1-800-677-1116). For suspected abuse or neglect, call the state survey agency; that's what those inspection teams exist for.
